MAP: Medically Assisted Procreation

Performed since 1990 in Angers at the Laboratoire Fonty, bd du Roi René, the activity of Medically Assisted Procreation (MAP) was transferred in 2007 to Laboratoire LabOuest Béclard, and then to the Angers Centre site in 2019.

Our Reproductive Biology Centre does specialized analyses of Spermiology by appointment, as well as specific blood tests related to fertility (hormones, virology, etc.) and genital specimen collection.

Doctors Vincent LOUSSOUARN, Gildas LOMONDAIS and Christophe MAY, specialists in Reproductive Biology, are responsible for these activities and approved by the Bio-Medicine Agency for the preparation of sperm for intraconjugal artificial insemination (IAI). They work in a network with all the gynaecologists in the region.

Depending on your doctor’s prescription, and the transport time of the specimen, the collection of semen can be done at the laboratory or at home.
In both cases, certain precautions must be observed for the collection under penalty of obtaining false bad results:

  • Sexual abstinence from 2 to 7 days.
  • Drink 1.5 litres of water the night before.
  • Urinate during the night if necessary, and one last time before collection.
  • Wash the hands and the penis (foreskin pulled back) with antiseptic soap, rinse well, dry well.
  • Collect all the semen in the receptacle, by masturbation.

In the laboratory, collection takes place in a specific room.

In case of collection at home, you must first come to the laboratory to obtain the special receptacle. The specimen should be kept at room temperature and brought to the laboratory as soon as possible (maximum 45 min).

This semen analysis essentially studies the count, the different types of motility and morphological characteristics of the spermatozoa.

This test looks for the presence of infection in semen. Some specific bacteria, like chlamydiae, can also be identified.

In the initial fertility test, a hormonal blood test is performed for women and sometimes for men. In women, the basal level of hormones is studied at the start of the cycle (2nd-3rd day): make an appointment at the laboratory from the first day of your period. For prolactin, a hormone which increases with physical activity and stress, placement of a venous catheter is recommended and a rest period of 20 minutes in the laboratory is necessary. In case of irregular cycles, a hormonal assessment is carried out in the second phase of the cycle (22nd-23rd day).

During ovarian stimulation, the growth of follicles is monitored by your gynaecologist using ultrasound, by our laboratory using hormonal dosages (estradiol, LH, progesterone).

In the context of Medically Assisted Procreation, legal serology testing (HIV, hepatitis B and C, syphilis…) will have to be done in the laboratory. In general, they are done during your consultation with the biologist at the LabOuest Angers Centre laboratory.

This analysis is subject to Prior Agreement.

This is a semen preparation test, the goal being to recover the maximum number of motile, rapid and typical sperm. The 24-hour sperm survival rate is also studied.

This test (TPC) is an exam to study the qualities of the cervical mucus in the peri-ovulatory period and assess the displacement of sperm from the spouse within this mucus. This examination is not painful, and is part of the initial fertility assessment.

Conditions for conducting the test :
The test should be performed one or two day(s) before the expected date of ovulation, determined by your gynaecologist relying on either the temperature curve or ultrasound.
In all cases, note the date of your last period (first day of full bleeding).
It is recommended that the man remain abstinent for 2 to 7 days, so that the sperm quality is optimal.
Intercourse should take place 6 to 12 hours before the mucus sample is taken at the laboratory. After this intercourse, the woman must lie down for around 20 to 30 minutes. The woman must not clean her vagina before the test, but may shower the rest of her body.

The sample itself :
It is taken in a gynaecological position, after placement of a speculum, by gentle suction.

The parameters studied are :
Opening of the cervix, the abundance, the spinnbarkeit (fibrosity), the manner of crystallization and acidity of the cervical mucus. We are looking for the possible presence of leukocytes which indicates inflammation. Each parameter is noted; we can then establish the Insler score. The motile and immobile sperm are also counted under a microscope.

We perform vaginal and urethral swabs to detect any infections. Some can cause infertility. Il est advised to make an appointment for these swabs. For urethral swabs, patients are asked to refrain from urinating for two hours before the examination.

The LabOuest Angers Centre laboratory is approved for the preparation of sperm for intra-conjugal artificial inseminations (IAI).

The preparation consists of retrieving the motile, rapid and typical spermatozoa; they will then be injected intrauterine.

The indications for IAI are discussed in clinico-biological meetings. Your gynaecologist will explain the clinical procedures to you and, during a consultation with the biologist in the laboratory, the technical methods of insemination will be presented to you. This consultation in the laboratory will also be the opportunity to establish the IAI file.

On the advice of your gynaecologist, make an appointment at the LabOuest Angers Centre laboratory.

During this consultation, the MAP legal file will also be established, which will be kept for 20 years at the LabOuest Laboratory.
It must include :
– Photocopies of identification cards,
– Photocopy of the family record book or, in case of cohabitation, a copy of the PACS (civil solidarity pact) or certificate of cohabitation issued by the town hall,
– The request for Medically Assisted Procreation dated from the first consultation for infertility for which insemination has been recommended,
– The approval at the national MAP register of the Biomedicine Agency
– The clinical summary sheet established by the gynaecologist,
– Laboratory results of health tests dating back less than 6 months for the first insemination, two results from health tests, one of which one must date back less than one year, for each subsequent insemination: Blood tests for HIV, hepatitis B and C, syphilis; and also, for women, blood tests for rubella and toxoplasmosis; the latter renewed regularly.
– Laboratory results of a spermogram, of a Migration-Survival Test and a sperm culture dating back less than 1 year.-Un Consent for artificial insemination dated the day of insemination and renewed with each attempt.

This analysis is subject to Prior Agreement.
The MAR-Test looks for the presence of antibodies in semen against the spermatozoa.

Retrograde ejaculation is ejaculation which, instead of going towards the outside, goes towards the bladder.
Examination consists of looking for the presence of sperm in the urine released immediately after ejaculation. The collection is done at the LabOuest Angers Centre laboratory. It is necessary to heed the following instructions:
– For the the 2 or 3 days preceding the collection, drink only Vichy Célestin water.
– On the day of the collection, empty your bladder and drink 200 ml of Vichy Célestin just before going to the laboratory for the collection.

For the investigations necessary for the diagnosis of infertility and for the treatment thereof, including by means of Artificial Insemination, you can, in consultation with your gynaecologist, request cover at 100% by Health Insurance (Article 322.3.12 of the Social Security Code).

Furthermore, to be reimbursed for certain specific examinations (Migration Survival Test, MarTest, Sperm Biochemistry), it is imperative to establish a “Preliminary Agreement” independently of any cover by supplemental insurance or at 100%. The Preliminary Agreement request is given to you by your gynaecologist. You must have it stamped at the Andebio Angers Centre Laboratory, before sending parts 1 and 3 to the medical adviser of your health insurance fund.

In any event, in order to avoid having to advance any costs, remember to bring all the necessary certificates of cover (Carte Vitale, Supplemental insurance or 100% reimbursement), as well as prescriptions from the gynaecologist, for your visits to the laboratory.
